The Village Point Rehabilitation & Healthcare team consists of the clinicians, rehab specialists, and caregivers dedicated to providing the most innovative, compassionate level of care you’ll find in Monroe Township, New Jersey. Following the Springpoint model for awareness and sensitivity, all associates receive ongoing training to stay alert to changes that may require adjustments in treatment. Meet the members of our executive leadership team:
Jay is responsible for the overall leadership and direction of Monroe Village and Village Point. With 18 years’ experience in the senior living industry, he served as Executive Director at Meadow Lakes, leading the team through capital improvements totaling more than $20 million to enhance amenities and infrastructure campuswide. The initiative improved communication with residents, resident satisfaction, and demonstrated repeated survey excellence in assisted living and skilled nursing neighborhoods, including advanced standing recognition for the community’s assisted living option.
Jay graduated magna cum laude from Muhlenberg College with a Bachelor of Arts in business administration/Bachelor of Arts in economics; is a summa cum laude graduate from Temple University with a Master of Business Administration in healthcare administration/Master of Science in healthcare financial management; and is a licensed nursing home administrator in the Commonwealth of Pennsylvania and the State of New Jersey. His industry affiliations include chair of the Springpoint Human Resources Advisory Board; Springpoint Ethics Committee member; and Advisory Board member for the Jersey College School of Nursing’s Ewing, N.J., campus.
Ms. DellaPietro is responsible for the day-to-day operations at Village Point including licensed Administrator of Record. She will continue the tradition of excellent clinical outcomes and exceptional customer service at Village Point.
She has been a part of the Springpoint family for over 30 years; she began her journey as a Charge Nurse. For the past nine years, LuAnn has served as the Director of Health Services for The Atrium at Navesink Harbor, a Springpoint Life Plan Community located in Red Bank, NJ. She is a trusted resource throughout Springpoint, serving on various corporate-wide committees regarding clinical care and service.
Dr. Nanavati is committed to a patient-centered approach to care. With 40 years of experience in the medical field, he has a private medical practice specializing in geriatrics and internal medicine. His services are exclusive to nursing homes, assisted living facilities, house calls, and hospitals. He has attending privileges at University Medical Center of Princeton, Robert Wood Johnson University Hospital, and Saint Peter’s University Hospital. His medical directorship includes the management of emergency rooms in several hospitals, with Robert Wood Johnson University Hospital accounting for a volume of 34,000 patient visits per year, having a down closing rate close to 0% all the time.
Ms. Chakraborty has 22 years of experience in the healthcare industry and 15 years of experience in the marketing business development and admissions process for several major companies. She’s part of the Reducing Re-hospitalizations initiative and Care Coordination meetings at Robert Wood Johnson University Hospital, CentraState Medical Center, and St. Peter’s University Hospital. Her experience also includes corporate training related to compliance, regulatory affairs, ethics, coaching and counseling, and WE CARE Sensitivity. Ms. Chakraborty earned double master’s degrees in business administration and clinical psychology, respectively. A certified social worker and certified dementia practitioner, she is a member of the Geriatric Advisory Council and a chairperson of Walk to End Alzheimer’s New Jersey.
Ms. Mennella has extensive experience in areas of speech, language, and hearing disorders; mechanisms including apraxia, dysarthria, articulation, auditory processing, cognitive-linguistic skills, and executive functioning; aphasia therapy; and dysphasia management. She earned her Bachelor of Arts in English, French, and linguistics from Drew University, and a Master of Arts in speech-language pathology and communication disorders from Kean University. She is certified in clinical competence by the American Speech-Language-Hearing Association, a licensed speech language pathologist, and a certified speech-language specialist in New Jersey.
Ms. Callahan-Rodriguez is responsible for managing the daily nursing operations of the Rehabilitation and Healthcare neighborhoods. She has served both communities, Monroe Village and Village Point for over ten years. An LPN in our Healthcare Clinic, she steadily advanced her education by obtaining her RN licensure. Her experience within the campus includes Clinic Coordinator, MDS Coordinator, Memory Care Director and Assistant Director of Nursing.
Recognized as a leader, she continues to ensure positive outcomes for residents and staff. Her medical expertise and understanding of customer service is an asset to Village Point.
