Helping Patients Manage Chronic Conditions.
We offer Remote Patient Monitoring (RPM) services for residents who are discharged home from sub-acute rehabilitation and have limited means to follow up with their primary care provider during the first few days or weeks of transition. These services are also available to any discharged resident who does not have a primary care provider in the community.
Village Point offers RPM services with the assistance of Clare Medical of New Jersey, a full-service medical provider that offers house calls and telehealth. RPM helps to monitor residents daily to help manage chronic conditions such as high blood pressure, congestive heart failure, and diabetes. Clare Medical delivers needed equipment such as blood pressure cuffs, pulse oximeters, weight scales, and blood glucose meters to residents’ homes during the first visit. Residents are continuously monitored, and should they require additional care, Clare Medical coordinates readmission pending clinical and financial clearance back to Village Point.
