Our cardiac management program provides interdisciplinary cardiac and palliative care for residents with a primary or secondary diagnosis of chronic heart failure and cardiac-related health issues. We manage our residents’ conditions and support their quality of life.
Our program includes medication management, imaging and lab tests, nursing care, diet, exercise, education, care transitions, and management of implantable devices. We provide the tools, information, resources, and education required for our residents to maintain an independent and healthy lifestyle. Board-certified interventional cardiologist Alpesh B. Patel, M.D., F.A.C.C., F.S.C.A.I., and his team from 247 Heart & Vascular of Kendall Park, are the consulting physicians who facilitate our cardiac management program. Our interdisciplinary team includes Village Point’s medical director, director of nursing, advanced nurse practitioners, registered nurses, registered dietician, program rehabilitation director, social worker, and case manager.
